Blastomycosis

Blastomycosis ( North American blastomycosis , Gilchrist's disease [3] , Gilchrist's blastomycosis ) is a fungal infection of humans and animals (in particular dogs, less often cats) caused by Blastomyces dermatitidis . Distributed in North America ; causes clinical symptoms similar to histoplasmosis [4] . The disease occurs in several endemic areas, most often in eastern North America, especially in the western and northern periphery of the Great Lakes basin, extends east along the southern bank of the St. Lawrence River and south to the center of the Appalachian Mountains in the east, in the Mississippi River Valley in the west . Isolated cases have been reported in continental Africa [5] , on the Arabian Peninsula and the Indian subcontinent .

Signs and Symptoms

Blastomycosis can occur in the following forms:

  • flu- like illness with fever , chills , arthralgia (joint pain), myalgia (muscle pain), headache and an unproductive cough that goes away after a few days;
  • acute illness resembling bacterial pneumonia , with fever, chills, productive cough and chest pain;
  • a chronic disease that resembles lung cancer or tuberculosis , with fever, productive cough, night sweats and weight loss;
  • a rapidly progressive and severe disease that manifests itself as ARDS with fever, shortness of breath , tachypnea , hypoxemia, and diffuse pulmonary infiltrates ;
  • skin lesions, as a rule, are asymptomatic, warts or small pustules may appear;
  • lytic damage to bones, can lead to bone pain or joint pain;
  • prostatitis may be asymptomatic or may cause pain when urinating;
  • hoarseness:
  • 40% of patients have weakened immunity and problems with the central nervous system in the form of possible brain abscesses , epidural abscesses or meningitis .
