Biography

From 1819 to 1823 he studied at the medical faculty of the University of Dorpat . In 1824 he defended his doctoral dissertation on the subject " Observationes et animadversiones de coxalgia" [1] . After graduating from the university, V.F.Sabler’s medical activity was concentrated in Moscow, where at the beginning of the 19th century he was the only psychiatrist serving disabled people, the psychiatric wards of the Old Catherine Hospital and the first private psychiatric hospital in Russia, opened in 1830 by Dr. Fedor Ivanovich Herzog , which since 1836 he began to manage [2] .

On October 1, 1828 he was appointed resident, and since 1832 he took the post of chief doctor of the house of the insane under the Catherine’s almshouse, or Moscow Dollhouse . In 1833, the home for the insane became an independent institution, separated from the almshouse , and in 1838 , at the suggestion of Vasily Fedorovich [3] , it was renamed the Moscow Transfiguration Psychiatric Hospital . Thanks to the reorganization carried out by Sabler, the chains binding the restless patients were destroyed. In 1834, he drew up a detailed regulation of the psychiatric hospital "On the activities of patients in the insane asylum", and also began to improve the conditions of patients. Workshops and a garden for occupational therapy were created. Also, patients were entertained by music and games. In November 1870 he left the service.

Scientific Contribution

V.F.Sabler is the author of more than 30 scientific papers on various problems of psychiatry. He gave a scientific justification for the need to separate the mentally ill into acute (they were treated with therapy and left in the hospital) and chronic (who were sent to the almshouse) depending on the characteristics of the course of their disease and showed differences in the therapeutic approach to them. However, Sabler believed that chronically ill patients also needed to be treated. Hydrotherapy , psychotherapy and the then advanced treatment of progressive paralysis with anti-syphilitic drugs were used to treat mentally ill patients [4] . According to Subler, mental illness is one of the protective properties of the body, allowing the body in many cases to deal with physical diseases.

Sabler’s concept of dealing with acute psychoses and chronic diseases is expressed as follows:

The doctor, using the debris of the destroyed building of reason, erects a new building from the old material.
