Septicopyemia

Septicopyemia (from other Greek septicopyaemia - causing suppuration, - pus and - blood) is a form of sepsis , in which, along with general intoxication of the body, the formation of metastatic abscesses in various tissues and organs.

Most often develops as the next stage of septicemia . Therefore, septicopyemia, as a rule, should be preceded by a period of primary infection and septicemia.

Symptoms and treatment of septicopyemia

Body temperature rises to 40 Β° C, repeated chills. The general condition of patients is severe, observed: adynamia, weakness, inhibited or agitated state. The skin is pale, characterized by cyanosis of visible mucous membranes , pain in muscles and joints. All patients have heart failure (tachycardia, tachypnea, deaf heart sounds). Blood pressure in half of the patients decreases.

The hemogram is characterized by moderate leukocytosis , sometimes leukopenia, neutrophilic shift of white blood; ESR - up to 40-65 mm / h. Every two out of three patients develop anemia. With repeated purulent metastases, a worsening of the hemogram and an improvement in remission are noted.

In patients with septicopyemia, purulent lesions of the kidneys, liver, heart, meninges and brain are observed. The multiplicity of lesions is the most characteristic manifestation of septicopyemia.

Diagnosing septicopyemia is easier than septicemia. Often, after a period of some improvement, chills resume, fever occurs, intoxication builds up, signs of damage to individual organs and systems appear. Cardiopulmonary insufficiency indicates the presence of pneumonia or endomyocarditis.

Kidney damage is manifested by chills and fever, pyuria, transient oliguria, and pain. Enlarged liver, icteric staining of the skin, pain indicate inflammatory changes in the liver. Persistent headache, lethargy or agitation, "dimming of consciousness", meningeal phenomena indicate the occurrence of purulent meningoencephalitis. There are no specific laboratory tests specific to septicopyemia.

Bacteremia during treatment with antibiotics can be detected only in 15-25% of patients.

Treatment of septicopyemia is carried out according to the sepsis therapy regimen with mandatory surgical treatment of metastatic purulent foci.
