Unloading diet therapy

Unloading and diet therapy (RDT) - restriction of the diet, or complete fasting for therapeutic or recreational purposes, used in conventional medicine [1] . There are guidelines for RTD, approved by the Ministry of Health and Social Development of the Russian Federation in 2005 [2] . In alternative medicine, some authors call “fasting and dietary therapy” their methods of “ therapeutic fasting ” [3] , which are not related to medicine.

RTT is used for hypertension [4] ; it is effective for hypertension-induced kidney diseases (hypertensive nephropathy ) [2] , for allergic diseases ( atopic dermatitis [5] , asthma [6] ), and chronic alcoholism and other diseases.

It has a powerful immunosuppressive effect, and also stimulates adrenal function and is used as one of the components of treatment for cancer [7] . The use of RDT for mental illness is assessed ambiguously and its effectiveness is controversial. Successful use of RTT in chronic fatigue syndrome [8] .

As a rule, unloading and dietary therapy is used in combination with physical therapy courses, walking. It is always used as an additional part of treatment, in combination with etiotropic (specifically aimed at curing a specific disease) treatment.

The use of RDT in official medicine of the USSR originates from the creation in March 1981 of the department of unloading and dietary therapy and therapeutic fasting at the 68th city clinical hospital in Moscow. The department was created on the initiative of the founder of the Soviet RDT school, Professor Yu. S. Nikolaev . For 17 years, Yu. S. Nikolaev supervised the activities of the department and led patients on its basis. The entire period of existence (1981-2006) was headed by Dr. V. Mironov .

Criticism

The first category nutritionist and author of more than a hundred works on nutrition, R. I. Vorobyov, in his book “Nutrition: Myths and Reality”, expressed the opinion that therapeutic fasting and RDT are phenomena of the same order, which are extremes [9] :

And even more so, for many days fasting is not indifferent to the body, even if we call it therapeutic or fasting-diet therapy (RDT). It is possible that with a number of diseases this method helps some patients; they use, say, insulin shock for schizophrenia or cancer, but in exceptional cases, for strict medical reasons and taking into account contraindications. However, insulin shock is not a mass treatment of diseases. The fasting method as a therapeutic agent became widely known thanks to its propaganda by people who are well able to spread their ideas.

On the contrary, Professor Yu. S. Nikolaev , author of the book “Starvation for Health”, in the article “The Essence and Development of Unloading and Dietary Therapy in the USSR”, published in the collection of scientific papers edited by academician of the Russian Academy of Medical Sciences, professor M.N. Volgarev and Doctor of Medical Sciences, Professor V. A. Maksimov, positively commented on self-medication using the RDT method:

The great popularity of the RDT method and the lack of hospitals using RDT quite naturally created the desire of many patients to conduct RDT on their own. In principle, self-medication for RTD is acceptable if the patient is sufficiently cultured and familiar with the RTD technique. Under these conditions, RDT can also be used for prophylactic purposes. Unfortunately, until now, many patients, turning to doctors for advice on RTT, most often receive a negative response.

Undoubtedly, the satisfaction of the population’s need for the use of RDT can be ensured only if this method of treatment is widely used in hospitals of various profiles. And for this, first of all, it is necessary to organize mass training of relevant medical personnel.
