Pharmacological action

It has antitumor activity and, in particular, is able to suppress the leukemia process. It is metabolized in the body with the formation of a substance that has the properties of an adenine antimetabolite (a substance that is close to natural metabolic products, but has the opposite effect) and can inhibit the development of the tumor process. The advantage of fopurin in the treatment of acute processes is the parenteral (bypassing the gastrointestinal tract) route of administration, which provides a more rapid onset of therapeutic effect.

Indications for use

Acute leukemia (a malignant tumor that originates from blast cells / bone marrow cells from which leukocytes, lymphocytes, red blood cells, etc.) are formed, and characterized by the appearance of these immature cells in the bloodstream): chronic myelogenous leukemia (blood cancer, in which the source the tumor process are granulocyte cells / bone marrow cells, from which the formed elements of the blood develop - leukocytes /); skin reticuloses (skin manifestations of hemoblastoses, a malignant tumor that developed from hematopoietic cells) and retinoblastoma (a retinal cancer).

Method of use

The drug is administered intravenously or intramuscularly. Immediately before use, the contents of the ampoule (40 mg of the drug) are dissolved in an isotonic sodium chloride solution, water for injection or 0.5% novocaine solution (with intramuscular injection). Fopurin dissolves slowly. At the time of intravenous administration of the drug and for 1-1 '/ 2 hours after it, the patient should be in a horizontal position (the possibility of orthostatic hypotension - a sharp drop in blood pressure when moving from horizontal to vertical). In acute leukemia, 40-120 mg are administered daily, depending on tolerability. Treatment begins with 40 mg and, with good tolerance, the dose is increased to 60, 80, 100 or 120 mg (usually 1.2-1.5 mg / kg). The course dose is 1-3 g. For children with acute leukemia, the drug is prescribed 5-30 mg once a day. During the course of treatment, 20–40 injections (500–700 mg of the drug) are produced. With skin reticulosis, fopurin is prescribed daily in increasing doses of 20, 40, 60, 80, 100 and 120 mg (intramuscular administration is permissible only up to 60 mg, administration of the drug at higher doses is accompanied by pain at the injection site). The course dose is 1.5-3 g. With retinoblastoma, fopurin is administered intramuscularly 1 time per day in an initial daily dose of 5 mg / kg for 5 days. Subsequently, with good tolerance, the dose is increased every 5 days by 5 mg, but not more than 40 mg per injection. During the course of treatment produce 10-60 injections. If necessary, with all the mentioned diseases, repeated courses of treatment are carried out.

Side

Possible headache, dizziness, mild dyspeptic symptoms (digestive upsets), drowsiness, burning sensation, heaviness and warmth in the limbs, intoxication, numbness of the skin in the back, allergic effects, orthostatic hypotension. With increased sensitivity of patients or with an overdose of fopurin, hemopoiesis may be inhibited: leukopenia (low white blood cell count) and thrombocytopenia (decrease in platelet count). In this case, appropriate measures are taken to stimulate hematopoiesis (hematopoiesis).

Contraindications

Diseases of the liver and kidneys with impaired function, pronounced leukopenia and thrombocytopenia, terminal state (the state of the body that precedes death).

Release form

In lyophilized form in ampoules of 0.04 g in a package of 10 ampoules.
