Biography
Ivan Ivanovich Grekov was born on March 5 ( 17 ), 1867 on the Tomilink farm in the Boguchar district of the Voronezh province to a family of Don Cossacks . There were six other children in the family besides him: four boys and two girls. After the premature death of the father, the mother was engaged in raising children. In 1876, Ivan entered the Novocherkassk gymnasium, after which he graduated with honors in 1885, he entered the Faculty of History and Philology of Moscow University , and a year later he transferred to the natural faculty. While studying at Moscow University, the Greeks had the opportunity to attend lectures by I. M. Sechenov and K. A. Timiryazev as a student. In 1890 he was arrested before the final exams for participation in the revolutionary movement, imprisoned in Butyrka prison and later expelled from the university. In the autumn of 1890, he was released and was given the opportunity to continue his education at the medical faculty of the University of Dorpat , where anatomist Augustus Rauber , the physiologist A. A. Schmidt , and the surgeon V. G. Tsege-Manteuffel were among the teachers at that time. In many ways, under the influence of the latter, I. I. Grekov developed a fascination with surgery. Together with Grekov, V. V. Smidovich studied at the university - later the well-known writer V. V. Veresaev , with whom he maintained friendly relations for many years to come. In 1894, Grekov graduated from the University of Dorpat, receiving the title of doctor [2] [5] [6] [7] [8] [9] .
In early 1895, Grekov moved to St. Petersburg and in March of the same year he was hired as an external physician [Comm 1] in the women's surgery department of the Obukhovsky hospital , the oldest and one of the largest hospitals in Russia, where he continuously worked for 39 years. performing over 40 thousand operations. In 1898, Grekov was admitted to the full members of the Surgical Society N. I. Pirogov. In 1901 he defended his doctoral thesis on the topic “Materials on the issue of bone defects of the skull and their treatment”. In 1903, Grekov became an assistant manager of the women's surgery department of the Obukhovsky hospital, and in 1905 he headed it, in 1912 he took the position of the head of the men's surgery department. In 1917, the male and female surgical departments of the hospital were merged, and the newly formed department was headed by I. I. Grekov. Since 1927, he became the chief physician of the Obukhov hospital, retaining the post of head of the surgical department. It was I. I. Grekov, who had considerable authority, achieved the cancellation of the decision to demolish the Obukhov hospital and carried out its large-scale reconstruction, bringing it to modern requirements. In 1928, at the Obukhov Hospital, on his initiative, a physiological laboratory was established, which, with Grekova’s active participation, was engaged in developing and introducing into clinical practice new physiological methods for examining patients [2] [5] [6] [7] [8] [10] .
At the same time, from 1909 for several years I. I. Grekov headed the Central Emergency Medical Station in St. Petersburg, and from 1915 to 1934 he held the position of professor and head of the department of hospital surgery at the Psychoneurological Institute (later 2nd Leningrad Medical Institute). In 1922, he became the editor-in-chief of the journal “ Herald of Surgery and Border Areas ”, the successor of the journal “Velyaminov’s Surgical Archive” journal, which he resumed after five years of neglect, and remained until the end of his life [2] [3] [6] [8] [9] [ 11] [12] [13] .
In 1931, after organizing the 3rd Leningrad Medical Institute on the basis of the Obukhov Hospital, I. I. Grekov headed the formed medical school. In 1932, for his great scientific, pedagogical and public merit, Grekov was awarded the honorary title of Honored Scientist of the RSFSR [6] [7] [9] .
I. I. Grekov died suddenly on February 11, 1934 during a scientific meeting in Leningrad. He was buried at the Communist site of the Alexander Nevsky Monastery [3] [11] . The headstone (sculptor I. V. Krestovsky, the architect Totes) was created in 1954.

Contribution to medical science
Peru I. I. Grekova owns more than 150 scientific publications on lung surgery, heart , abdominal organs , wound healing . Among them, of special importance are the works “On stitching wounds of the heart” (1903), “On the treatment of peritonitis” (1914), “On the treatment of rheumatic aneurysms with vascular suture and vascular transplantation” (1916), “Surgical treatment of cancer tumors, injuries and other pancreatic diseases "(1922)," On the treatment of injuries of the chest cavity "(1924)," On the excision of gangrenous hernia ent bloc "(1926)," On errors in surgery "(1927) [2] [12] .
In 1909, he was one of the first in the Russian Empire to introduce into practice the method of treating the surgical field using iodine tincture proposed in the autumn of 1908 by the Italian surgeon Antonio Grossich (Antonio Grossich) and consisting in four-time lubrication of the skin with 10% alcohol solution of iodine (5 minutes before the operation, just before the skin incision, before suturing the skin, after suturing). In April 1909, Grekov published in the medical newspaper “ Russian Doctor ” extensive work on this topic, which contributed to the widespread introduction of this new method in surgical practice [6] [15] .
To abdominal surgery
The greatest recognition of I. I. Grekov was achieved thanks to his scientific work in the field of abdominal surgery . Already in 1900, a young doctor published a paper on the treatment of fecal fistula by turning off the intestine. Greeks, based on the enormous material of the Obukhov hospital in the field of surgical treatment of intestinal obstruction , proposed to carry out early surgical interventions for this diagnosis, often fistulas and correct the treatment in the postoperative period, taking into account biochemical blood parameters . Greeks contributed to the widespread introduction in the diagnosis of acute intestinal obstruction symptom Obukhovsky hospital , consisting in balloon-shaped swelling of an empty ampoule of the rectum against the background of the anus , determined by finger study [6] [16] .
Grekov's deep study of the problems of intestinal obstruction showed the need for the development and implementation of new surgical interventions, mainly in case of colon bowels, and therefore in 1910 he was offered the invagination operation of the sigmoid colon , which had become dead when it was turned into a diverting part, later called “Greeks I ". The lack of complete satisfaction of the author and other surgeons with the results of treatment using this method led to I. I. Grekov's search for new, more advanced methods. As a result, in 1924, they were offered a double-stage resection of the sigmoid and descending colon, which, by its simplicity, supplanted the previous technique and made surgical treatment safer by entering the guidelines for operative surgery called “Greeks II” [5] [7] [ 9] [16] [17] .
Grekov promoted early surgery for acute appendicitis , which was reflected in his speeches at the VII and XII congresses of Russian surgeons. For many years he has been studying the diffuse peritonitis , developing more advanced methods of his surgical treatment. At the XII-th congress of Russian surgeons (1912), he delivered a keynote address "On the treatment of peritonitis." In his report, based on more than 200 observations of diffuse purulent peritonitis, Grekov comprehensively substantiated the indications for timely surgical treatment of diseases that can lead to peritonitis, and elaborated on the principles and techniques of the operation. He elaborated the method of tamponade of the abdominal cavity, and also determined the indications for it in peritonitis. This report he subsequently finalized and published in the journal "Russian Doctor" in 1914 [6] [7] .
I. I. Grekov determined the functional and reflex interrelationship between appendicitis and peptic ulcer disease , the Bauhinia valve and the gatekeeper , indicating that in diseases of the cecum , disorders of the motor and secretory functions of the stomach are possible; proposed a modification of the resection of the intestine with strangulated hernias [2] [5] .
Greeks first in the Russian Empire successfully completed the removal of the pancreas . In 1913, at the XIII Congress of Russian Surgeons, he gave a report “To pancreas surgery”, in which he reported a successful case of resection of a significant part of the pancreas for cancer, leaving an insignificant part of the gland in the form of a strip on the duodenum . The successful outcome of the operation allowed Grekov to express the hope that in the future surgical treatment of pancreatic cancer will be possible. It is noteworthy that the patient lived for 16 years after this operation [6] [7] [12] .
In cardiovascular surgery
I. I. Grekova rightly attributed to the luminaries of cardiovascular surgery . On July 8 ( 21 ), 1903 , the Greeks was one of the first in Russia to successfully complete the stitching of a stab wound in the heart. It is noteworthy that by the end of 1903, seven surgical interventions for heart injuries were performed in the Russian Empire, however, only two patients recovered, one of which was a patient operated by Grekov. In 1904, Grekov published in the Russian Surgical Archive magazine an article entitled “Three cases of suturing the wound of the heart,” in which he summarized the results of surgical treatment of three patients in the surgical department of the Obukhov hospital, and also analyzed 60 observations of suturing the wound of the patient, collected by him in the world medical literature. In this work, Grekov made bold proposals for treating patients with a wounded heart. For all his surgical practice, Grekov personally performed 6 surgeries for injuries of the heart. In 1928 he was the first in the history of domestic surgery to open the cavity of the right ventricle of the heart in order to remove bullets from it, and in 1929 presented this case in the article “Ventriculotomia cordis explorativa”. In order to revise, he was one of the first in the world to introduce a metal instrument into the cavity of the right ventricle of the heart and concluded that such manipulation was harmless [2] [4] [5] [6] [8] [9] [12] [18] [ 19] .
To Thoracic Surgery
I. I. Grekov was a supporter of the wait-and-see tactics [19] and the conservative treatment of penetrating chest wounds, the use of pneumopexy with their complication with open pneumothorax [2] [5] .
In 1915, he successfully performed the operation of Roux-Herzen in a 9-year-old boy who suffered from a post-burned stricture of the esophagus and had undergone a gastrostomy performed at another clinic. At the same time, Grekov made two deviations from the initial technique of P. A. Herzen : he placed the graft from the small intestine into an open subcutaneous tunnel under visual control and connected the graft to the esophagus at the second stage in order to speed up the restoration of normal swallowing. In the third stage, he connected the transplant with the stomach by placing the side and side of the anastomosis, and then closed the gastrostomy [20] . According to B. A. Petrov, by 1915, only four surgeons — P. A. Herzen, Alben Lambott , Cesar Roux, and I. I. Grekov [5] [6] [21] - managed to complete the small intestinal plastics of the esophagus all over the world . . The favorable long-term result of this operation Grekov showed after 6 years [22] .
The Greeks were the first in Russia in 1922 to perform the Giano-Galperna operation, which consists in creating an artificial esophagus by cutting out the gastric tube from the greater curvature of the stomach with the base in its bottom [23] [24] .
In Traumatology
Already in 1898, in the journal “The Chronicle of Russian Surgery ”, Grekov published a large-scale study “On the closure of cranial defects with calcined bone”, which formed the basis of his doctoral thesis “Materials on the issue of bone defects of the skull and their treatment” (1901). In his dissertation work, he showed that the use of calcined bone contributes to the closure of not only fresh, but also old defects of the skull bones. This study made a significant contribution to domestic surgery, since at that time the replacement of defects of the skull in particular and bone defects in general was one of the urgent surgical problems [7] .
During the First World War, I. I. Grekov began to actively engage in surgical treatment of bone fractures . During the period of hostilities, the question of the most appropriate treatment of severe purulent arthritis became topical, and I. I. Grekov convincingly proved that extensive resection of the knee joint makes it possible to achieve good results. Grekov proposed a modification of the operation when the cruciate ligament of the knee joint was damaged : a method of restoring the cruciate ligament of the knee joint by free transplantation of the broad fascia of the thigh [2] [7] .
Social activities
As early as 1898, I. I. Grekov became a full member of the N. I. Pirogov Surgical Society. Since 1911, Grekov was the vice-chairman, and from 1919 - the chairman of this society; since 1920 - honorary member and honorary chairman of the Surgical Society N. I. Pirogov. He contributed to the preservation of the museum in the memory of N. I. Pirogov at the Pirogov surgical society in Leningrad [2] [6] [8] [9] [12] [25] .
In 1922, with the active participation of Grekov, Pirogov’s Surgical Society after 5 years of oblivion caused by the October Revolution , the Civil War and the ensuing blow in the country, with the support of state institutions, resumed the issue of the Velyaminov Surgical Archive under the title The Bulletin of Surgery and Border Areas. The chief editor of the renewed journal was I. I. Grekov, who not only edited articles, but also determined the topics of the journal, dealt with its current affairs and directed its work. Initially, the journal was funded by the state, but from the fifth volume in 1925, The Bulletin of Surgery and Border Areas became the organ of the Pirogov Surgical Society, which was not able to fully cover the costs of publication. In connection with this, I. I. Grekov and a number of other famous professors of that time took upon themselves the material burden of publishing the Vestnik. Grekova also laid down his concerns on supplying the printing house with paper [2] [3] [6] [9] [26] [27] [28] .
Evidence of recognition of I. I. Grekov’s merit before national surgery is his election as chairman of the XVIth All-Russian Congress of Surgeons, held in 1924 [2] [5] [7] .
In 1928, I. I. Grekov, together with N. N. Bolyarsky and N. K. Lysenkov, retained from destruction the tomb of N. I. Pirogov in the village of Vishnya of the Vinnytsia region , which allowed later to create there the memorial museum of N. I. Pirogov in 1947 year [6] [8] .
Memory
- By the decree of the Presidium of the Leningrad Council of the RKKD, dated June 4, 1934, the journal “Journal of Surgery and Border Areas” was renamed to “Journal of Surgery named after I. I. Grekov” in memory of I. I. Grekov [3]
- The main building of the former Obukhov Hospital in St. Petersburg, 106 Fontanka Embankment, where I. Grekov worked, in 1969 installed a marble plaque by the work of architect I.P. Pasternak with the text: “Here, from 1895 to 1934 . worked an outstanding surgeon, Professor Ivan Ivanovich Grekov " [29]
- The department of faculty surgery of the North-Western State Medical University named after I. I. Mechnikov was named after I. I. Grekov [13]
Medical terms with the name I.I. Grekov
- Grekov's modification :
- Kalba surgery modification - surgery for portal hypertension : “windows” are cut in the peritoneum and oblique muscles of the side walls of the abdomen to the subcutaneous fatty tissue [30]
- modification of the Ru – Herzen operation — surgery to replace the esophagus during a burn: the small intestine is placed in an arcuate skin flap, which is closed by suturing the edges of the severed skin wound; thereafter, an anastomosis is imposed between the intestine and esophagus on the neck, and the next stage is an intestinal graft stitched to the stomach; removal of mucous membrane according to Sapezhkov in the blind part of the esophagus [20]
- Grekov's operation :
- surgery for a pancreatic tumor , is to remove it completely [31]
- surgery for strangulated umbilical hernia with signs of phlegmonous - gangrenous changes: a circular incision is made, penetrating into the abdominal cavity outside the infringing ring, at the base of the hernial protrusion; without opening the strangulated hernia, it is excised within the limits of healthy tissues, after which it is made of plastics of the hernial ring [32]
- The operation “Greeks I” (the method of Grekov I of removing the sigmoid colon) is a surgical operation consisting in invagination of the necrotic when turning the loop of the sigmoid colon into the preserving viability of its part with the expectation of subsequent rejection and discharge through the rectum: the portion of the sigmoid intestine to be removed, separates from the mesentery, evaginates beyond the stretched anal orifice, is resected, and the ends of the intestine are sutured with a circular suture with stitching it to the skin [33]
- Operation "Greeks II" (method of Grekov II sigmoid colon, two-stage resection of the sigmoid colon according to Grekov) - a surgical operation of two-stage resection of the sigmoid colon, used for its turning, which consists in imposing a bypass anastomosis between the resulting and the outlet of the intestine within healthy tissues with breeding out through an additional incision of a dead intestinal loop, which is cut off after a few days with a stitching of a stump with a deaf three-story seam; stumps are drawn into the abdominal cavity in 3-4 weeks [33]
- The Swenson-Hiata-Grekov operation is a two-stage surgical operation used in Hirschsprung's disease , which consists in intraperitoneal mobilization of the sigmoid and rectum (first stage), evagination of the mobilized part through the rectum and its extraperitoneal resection, including the aganglionic area, with the anastomosis, and an overlap of the anastomosis, with an anastomosis, with an overlap of anastomosis and an extravascular anesthesia; sigmoid and rectum (second stage) [7] [16] [17] [34] [35]
- Grekov's way :
- Bauhinoplasty method for bauginospasm: the ileum anterior wall is dissected longitudinally, a grooved probe is inserted through the Bauhinia flap into the lumen of the cecum , through which the anterior wall of the cecum is dissected in the transverse direction along with the flap; then the wound is stitched in the transverse direction so that the flap does not fall into the seam [36]
- method of treating elephantiasis: in order to improve lymphatic drainage from the subcutaneous tissue, a flap is attached at the leg from the wide fascia of the thigh to the periosteum of the femur [37]
- method of bowel emptying with high intestinal obstruction : the contents of the overflowed intestine are squeezed out with fingers towards the stomach (against peristalsis ), and the emptying of the stomach is performed through a probe [36]
- method of emptying the intestine in case of low intestinal obstruction: the contents of the overflowing intestine are squeezed out with fingers towards the bottom, along the direction of the motility [36]
