This station was opened on August 23, 1920 . It is represented by two side platforms and two paths between them. The track walls are tiled with a brown-green decorative line and original 1920s inserts. They depict the letter "W": the first letter of the station name. There are also mosaics with the full name of the station "WINTHROP ST". In the 1950s, both platforms were extended to the north, taking into account the increased length of IRT trains. In this extended part there are station names in the form of simple "WINTHROP STREET" labels. The beam columns of the station and the mezzanine are painted green and they also have the name of the station in the form of black plates. The original signs in the form of mosaics to Downstate Medical Center and Kings County Hospital have survived to our time.
Each platform has its own payment area. Therefore, a free transition between directions is impossible. The exit from the platform towards Manhattan consists of several turnstiles located at the platform level, and a staircase leading to the southeast corner of the intersection of Winthrop Street and Nostrand Avenue. The south platform pay zone is located on a small mezzanine, from where two stairs go to the platform. On the mezzanine there are two turnstiles for exit only and one full-height turnstile. A staircase emerges from the mezzanine to the southwest corner of the intersection of Parkside Avenue and Nostrand Avenue. This exit is one block south of the exit from the northbound platform.
