It originates in the area of the Cumberland Plateau, in the southeast of Magoffin County, and flows in a northwesterly direction, through the cities of Saliersville and West Liberty. In Rouvan County, on the territory of the Daniel Bun national forest, forms the Cave Run reservoir. Below the reservoir, about 13 km north-west of Carlisle, the river receives the tributary Fleming Creek. Approximately 16 km north-west of the city of Mount Oliver takes the tributary of North Fork, and to the south of the city of Falmaus - the tributary of South Fork. It flows into the Ohio River opposite the city of Cincinnati (Ohio), near the towns of Covington and Newport.
