Septic shock

Septic shock is a life-threatening complication of severe infectious diseases, characterized by a decrease in tissue perfusion , which disrupts the delivery of oxygen and other substances to the tissues and leads to the development of multiple organ failure syndrome . Most often, septic shock occurs in children, people with immunodeficiency , the elderly and abortion. The probability of death is 25-50%. [one]

Diagnostic criteria for septic shock

Septic shock is a subclass of distributive (distribution) shock , a condition in which the pathological distribution of blood flow through the microvasculature leads to inadequate blood supply to body tissues, ischemia and multiple organ failure .

Septic shock can be defined as persistent septic-induced hypotension, which does not resolve despite the implementation of infusion therapy [2] .

Septic shock is a stage of a systemic inflammatory response syndrome (SIR), which develops when the body is unable to stop a local infection. The massive release of inflammatory response mediators during the generalization of the inflammatory response causes pronounced vasodilation, a decrease in blood pressure, and, as a result, tissue perfusion pressure, which leads to tissue hypoxia and multiple organ failure.

According to current guidelines, sepsis is defined as an infectious disease (suspected or confirmed), combined with systemic manifestations. [2] These manifestations include:

  • Tachypnea > 20 per minute or PaCO2 level below 32 mmHg. Art.
  • The number of leukocytes increased more than 12 Γ— 10 9 / l or decreased below 4 Γ— 10 9 / l;
  • Tachycardia > 90 beats per minute;
  • Body temperature > 38.0 Β° C or <36.0 Β° C;

Systemic shock is diagnosed in the presence of the above symptoms and persistent non-stopable sepsis-induced hypotension - systolic pressure below 90 mm Hg. Art., average blood pressure below 70 mm RT., or due to sepsis, a decrease in blood pressure by 40 mm RT. Art. and more [2] .

Septic shock abortion

Septic shock occurs immediately after abortion, as a response to bacterial infection. The infection is usually localized, but if it enters the bloodstream, sepsis occurs. With sepsis, blood pressure drops sharply, leading to septic shock.

In the event that the shock is caused by an independent abortion at home, a complete hysterectomy may be required to remove the source.
