Elyner (Elyner) - a removable orthodontic apparatus , which is a special caps on the teeth of polycarbonate . Aligners are used to correct the position of teeth in people in dental, orthodontic, therapeutic and other departments of hospitals, clinics with inpatient care. For the first time, the aligners were invented by the American company Align Technology in 1998. [one]
The design of the product provides for a weak constant pressure on the tooth, causing the resorption of the bone tissue of the alveoli in the direction of movement of the tooth and the formation of bone tissue in the opposite direction. As a result, a gradual movement of the tooth along a predetermined path is ensured.
Aligners have an aesthetic appearance and can be easily removed, providing access to the implementation of oral care procedures.
Aligners are made of durable polymer plates, made by molds.
Products are not subject to any staining. The product is a one-piece design, which, if necessary, is easy to remove for cleaning, both the product itself and the oral cavity.
The choice of various options for the correction of bite and dental position should be made in accordance with the opinion and recommendations of a specialist (orthodontist) [2] on the basis of the performed examination, which allows to determine the cause and degree of anomaly severity. Only a specialist will be able to correctly select the solution that will be sent to the patient's recovery.

Advantages and disadvantages of the traditional bracket system
Benefits
- It is more aesthetic because it is transparent and has a small thickness.
- Individual production of the patient's molds
- This product is in contact with the crown of the teeth on all sides, which improves control over the movement of teeth
- Simplified oral hygiene , since the device is removable
- Less trauma to the soft tissues of the mouth
- Reduced time spent at the orthodontist dentist
Disadvantages
- the overwhelming majority of manufacturers have a narrow range of indications for use (only dental alveolar forms of occlusion anomalies ). Invisalign, today is the only manufacturer whose aligners allow to successfully treat all cases of occlusion violation, which are treated with braces systems. This is achieved through the use of 3 patented know-how: material SmartTrack; SmartForce - directional forces due to the use of Optimized Attachments, Bite Ramps, Power ridge, and so on; SmartStage - treatment protocols programmed and calculated by unique Software - ClinCheck Pro. This explains the relatively high cost of treatment.
- Relatively high cost
Manufacturing
Products are manufactured using thermoforming technology in extrusion machines under pressure or in vacuum, using polymer plates of various thickness, which, when heated, acquire plasticity and allow duplication of real or modeled objects of various shapes by extrusion in an apparatus. In this case, the object of duplication is plaster, composite or plastic models of the jaws, made on the patient's individual impressions. Today, the production of e-liners is carried out in the USA, Mexico, Korea, Italy, Germany, England. Since 2012, after the expiration of the patent, the steel liners have been manufactured in Moscow.
Stages of a doctor's work
To place an order for the manufacture of e-liners, the doctor must fulfill the following mandatory conditions:
- Remove prints from polyvinylsiloxane material (A-silicone) on plastic spoons. These will be working prints.
- Remove diagnostic prints from any impression material, make diagnostic plaster models (any plaster version).
- Make an occlusal pattern (from any material).
- To prepare an orthopantomogram (on any medium - paper or electronic) is a prerequisite; lateral / direct projection X-ray diffraction pattern - at the request of the orthodontist of the manufacturer.
- Send 4 (four) photographs of the patient’s face (shoulders, ears that are not covered by hair should be visible):
- Fas without a smile
- Fas with a smile
- Profile without a smile
- Profile with a smile
- Send 5 (five) photographs of the patient's dentition:
- Occlusal surface of the upper dentition
- Occlusal surface of the lower dentition
- Closed dentitions in the usual occlusion of the right, left, front
- Fill out the order form, which reflects the basic information about the patient and the doctor's wishes regarding the treatment plan.
- Send all of the above to the courier company.
- The orthodontist-technologist evaluates each clinical case with a view to the possibility of using elieners. After an affirmative answer, the working impressions are sent to production, where they are either scanned, or dynamic plastic models are made according to them (depending on the technology used by this company).
Tooth Movement Technology
Scanning
The mold is scanned on a 3D scanner , a special computer program turns the scan into a three-dimensional model of teeth. The technologist moves the teeth along the trajectory previously set by the doctor, then plastic models are made on the 3D printer and molded on the tray. Advantages: fully automatic process. Disadvantages: large errors (discrepancy between the virtual and real results).
Dynamic models
The cast is poured with a special plastic, a proprietary dynamic pin is installed in the projection of each tooth. After curing of plastic, the model is segmented into separate teeth (the model base remains intact). The technologist uses the key to activate the pins, moving them along the trajectory set by the doctor. Pitch moving pitch 0.1-0.25 mm. After each activation Kappa is pressed. Advantages: high accuracy of the result. Disadvantages: human factor.
Indications and contraindications for use
Indications

Contraindications
- Skeletal forms of anomalies of the dentition
- Periodontal disease in the acute phase
- Diseases of the temporomandibular joint
- The destruction of bone tissue of the alveoli in 1/2 the length of the tooth root
