General information

Crystalline powder, white with a yellowish shade. Practically insoluble in water, sparingly soluble in ethanol .

The drug was obtained as a result of an atypical chemical transformation of the diazepam molecule. Like other benzodiazepines, it has tranquilizing activity, but differs in a number of properties: it does not cause drowsiness, does not have muscle relaxant and anticonvulsant effects. Due to the significant difference in the chemical structure from other benzodiazepines, it almost does not cause cross-habituation and withdrawal syndrome with proper control during administration and compliance with dosages.

Application

It is used for neurosis and neurosis-like conditions accompanied by tension, autonomic disorders, moderate fear, as well as for conditions characterized by apathy, low activity. It is also indicated for alcohol withdrawal syndrome, does not potentiate the effect of alcohol.

In the treatment with tofisopam, the development of increased irritability, dyspeptic symptoms, allergic reactions (skin itching, rash) is possible, which requires discontinuation of the drug or dose reduction. The drug should not be prescribed to women in the first 3 months of pregnancy ( teratogen ). Care must be taken when prescribing the drug to persons whose work requires a quick mental and motor reaction.

Contraindications

Hypersensitivity (including to other benzodiazepines), psychoses and psychopathies with severe psychomotor agitation, aggressiveness or deep depression; decompensated respiratory failure, sleep apnea syndrome (history), simultaneous use with tacrolimus , sirolimus , cyclosporine ; pregnancy (I trimester), breastfeeding, age up to 18 years.

Application restrictions

Decompensated chronic respiratory distress syndrome, a history of acute respiratory failure, angle-closure glaucoma, epilepsy, organic brain damage (e.g., atherosclerosis).

Side effects

From the nervous system and sensory organs: headache, psychomotor agitation, irritability, aggressiveness, increased irritability, sleep disorder; sometimes - confusion, seizures in patients with epilepsy.

From the respiratory system: respiratory depression.

From the digestive tract: loss of appetite, nausea, dry mouth, gastralgia, dyspepsia, increased gas separation, constipation; in some cases, congestive jaundice.

From the musculoskeletal system: muscle tension, muscle pain.

Allergic reactions: itchy skin, exanthema.

Precautions

If you experience an allergic reaction or serious sleep disturbances, you should stop taking it. To prevent the disorder of falling asleep, it is recommended to take the drug for the last time no later than 15-16 hours.

It should be borne in mind that in patients with mental retardation, elderly patients, as well as those with impaired renal and / or liver function, side effects can be observed more often than in other patients.

Tofisopam is not recommended for chronic psychosis, phobia, or obsessive conditions. In these cases, the risk of suicidal attempts and aggressive behavior increases. Therefore, tofisopam is not recommended as a monotherapy for depression or depression, accompanied by anxiety.

Caution is required in the treatment of patients with depersonalization, as well as organic brain damage (such as atherosclerosis). Constant medical supervision is required when prescribing to patients with myasthenia gravis.

In patients with epilepsy, tofisopam may increase the threshold for convulsive readiness.

Tofisopam does not cause a significant decrease in the ability to concentrate. The question of the possibility of driving vehicles is decided after evaluating the individual patient's response to the drug.
