Content
Creation History
The preparation was synthesized at the Laboratory of Nitrogen-Containing Compounds (Head - L. I. Khmelnitsky) of the ND Zelinsky Institute of Organic Chemistry [1] . The tranquilizing ability of the compound was revealed by Irina Evgenievna Zimakova at the Department of Pharmacology of Kazan State Medical Institute , which was engaged in the study of poly nitrogen nitrogen heterocycles, including derivatives of bicyclic bismoureas [2] . The drug, called mebicar, was submitted to the Pharmaceutical Committee of the Ministry of Health of the USSR to obtain permission for clinical trials, and in 1978, by order of the USSR Ministry of Health N 868 it was approved for wide clinical use as a tranquilizer and appeared in the pharmacy network [3] .
The industrial production of mebicar was established at a pharmacological factory in Riga .
Since 2000, it has been produced by the Kazan plant "Tatkhimpharmpreparaty" [4] .
In 2002, at the All-Russian competition, the drug received the diploma " 100 best goods of Russia " [2] .
Pharmacological action
It has moderate tranquilizing activity; It does not have a muscle relaxant effect and does not interfere with the coordination of movements . It does not have a hypnotic effect, but it enhances the effect of sleeping pills and improves sleep during its disturbances.
Mebikar acts on the activity of structures included in the limbic-reticular complex, in particular, on the emotiogenic zones of the hypothalamus, and also affects all 4 main neurotransmitter systems: GABA -, choline -, serotonin - and adrenergic , contributing to their balance and integration, but does not have a peripheral adrenonegative effect.
In addition to calming, it has a nootropic effect. In clinical studies, it was found that mebicar increases logicality, coherence and speed of thinking, improves attention and mental performance , without stimulating the symptoms of productive psychopathological disorders: delirium, pathological emotional activity.
Indications
The following indications for the use of mebicar have been registered [5] :
- with neurosis and neurosis-like conditions occurring with the phenomena of irritability , emotional excitability, anxiety , fear (in particular, in patients with alcoholism during remission );
- hypomanic and anxiety- delusional states without psychomotor agitation and serious behavioral disorders (including schizophrenia, involutional and vascular psychoses);
- residual effects after acute psychoses with phenomena of emotional instability, productive symptoms ;
- chronic verbal hallucinosis of organic origin;
- in the treatment of nicotine withdrawal symptoms .
It can be used in combination with antipsychotics and other tranquilizers .
Dosages and duration of use
Assign inside tablets of 0.3-0.6 g 2-3 times a day (regardless of food intake). The maximum single dose is 3 g, the daily dose is 10 g. The maximum concentration of the active substance in the blood is reached 0.5 hours after taking the drug and a high level persists for 3-4 hours, then gradually decreases. The duration of the course of treatment is from several days to 2-3 months. [five]
To reduce the craving for smoking, tobacco is prescribed at 0.3-0.9 g per day for 5-6 weeks. [five]
The active substance is not metabolized and does not accumulate in the body.
Precautions
It should not be used while drivers of vehicles and people whose profession is associated with increased concentration of attention [5] .
Side effects
When taking mebicar allergic reactions ( skin itching ) are possible. In such cases, it is necessary to cancel the drug. Possible decrease in blood pressure ( hypotension ) and hypothermia (decrease in body temperature, by 1-1.5 ° C), which usually normalize themselves. There are also drowsiness , weakness , dizziness , dyspeptic symptoms. [five]
