The Monokashi Basin is 1,927 km², Pennsylvania accounts for 25%, the rest is in Maryland. The largest city in the basin is Frederick . Most of the basin is used for agriculture; the river, especially its lower course, is polluted from effluents containing fertilizers, manure and pesticides. However, Monokashi is officially considered the "wild and picturesque river" ( Maryland Wild and Scenic River).
One of the local attractions is the aqueduct through Monokashi , through which the Chesapeake - Ohio canal passes over the river.
In 1864 , during the Civil War , an important battle took place on the shores of Monokashi, the so-called "battle that saved Washington ."
