Nitroglycerin preparations have an antianginal, vasodilating and coronary dilating ( vasodilating ) effect. Nitroglycerin is a colorless oily liquid, poorly soluble in water, good in alcohol , ether , chloroform [1] . It is prone to detonation, therefore, in its pure form is not used as a medicine.
Due to the short-term vasodilating effect due to the activation of the enzyme guanylate cyclase and a decrease in the level of cytosolic calcium, it is mainly used for the rapid relief of angina attacks [3] .
History

Nitroglycerin was discovered in 1847 by Ascanio Sobrero , who continued the work of his teacher, Theophile-Jules Peluse in Turin [4] . Sobrero was also the first to draw attention to a severe headache caused by a test on the tongue of small amounts of nitroglycerin.
In 1849, Konstantin Goering Studying the pharmacological effect of nitroglycerin on volunteers, he confirmed that its use quite naturally causes a headache. Goering suggested using nitroglycerin as a homeopathic remedy for headaches, in accordance with the doctrine he supported, " treat like-like like " [5] . The name "glonoin", given by Goering to nitroglycerin, has been preserved in homeopathy.
In 1851, Alfred Nobel joined the studies of Pelus, who was able to assess the potential use of nitroglycerin as an explosive . Having overcome the problems of handling extremely explosive nitroglycerin and having patented his method of manufacturing explosives, he began its industrial production in Sweden. Ironically, Nobel rejected the use of nitroglycerin as a medicine, although he himself suffered from acute attacks of angina pectoris.
In addition to nitroglycerin, in the mid-19th century, scientists from Great Britain became interested in the recently discovered amyl nitrite , recognizing it as a potent vasodilator. Thomas Lauder Brenton , one of the founders of modern pharmacology, first used this substance to relieve angina attacks in 1867, however, noting the body's resistance to its action with frequent dosage of the drug. It also became clear over the years that the effect of amyl nitrite is shorter than that of nitroglycerin, and the side effects in the form of a headache are stronger, and many other undesirable reactions were also found.
William Murrell first used nitroglycerin for angina pectoris in 1876, although at that time nitroglycerin was listed in the British Pharmacopoeia only as a medicine for high blood pressure [5] . Murrell, a medical practitioner who worked in two London hospitals at the same time, experimentally determined the optimal dosage from the ratio of effectiveness and safety: 1% alcohol solution of nitroglycerin in 3 drops with half an ounce of water three times a day. In this form, the drug acted much longer than amyl nitrite, and more gently. Murrell's publication [6] , which summarized the results of several years of research, first on itself, and then on patients with angina pectoris, was included in the golden fund of medicine.
A year later, pharmacist William Martindale a solid form of the preparation was developed: 1/100 grains of nitroglycerin in chocolate - which, in addition to ease of use, had better digestibility. In 1882, Parke, Davis & Co. began to produce nitroglycerin in five different dosages already [7] .
The nitroglycerin industry experienced its heyday in 1900, while the workers in these enterprises were exposed to significant exposure to organic nitrate vapors. The effects were manifested in the form of addiction to nitrates, as evidenced by the symptoms of βMonday diseaseβ ( Eng. Monday disease ), disappearing by the end of the week, and withdrawal symptoms - Sunday heart attacks ( Eng. Sunday Heart Attacks ) [5] . All this prompted medical scientists to further research on these substances.
At the beginning of the 20th century, scientists tried to establish the principle of action of nitrate-containing substances in vitro , however, despite efforts, there was no significant shift in understanding the pharmacokinetics of these drugs [5] .
Only in 1977, Ferid Murad established the principle of release of nitric oxide (NO) from nitroglycerin and its effect on vascular smooth muscle [5] . In 1980, Robert Furchogt and John Zawadski discovered the role of endothelium in acetylcholine- induced vasorelaxation , and in 1987, Luis Ignarro and Salvador Moncada established that nitric oxide is the endothelial-derived relaxing factor, EDRF) [5] .
For clarification of the role of nitric oxide in the regulation of vascular tone, Robert Furchgott, Luis Ignarro and Ferid Murad received the 1998 Nobel Prize in Physiology or Medicine .
Other organic esters and inorganic nitrates can be used to relieve symptoms of angina pectoris, however, the extremely fast action and the recognized effectiveness of nitroglycerin make it the main tool for stopping angina attacks [5] .
Pharmacological action
Venodilating agent from the group of nitrates. Nitrates are able to release nitric oxide (II) from their molecule , which is a natural endothelial relaxing factor - a mediator of direct activation of guanylate cyclase . An increase in cGMP concentration leads to relaxation of smooth muscle fibers (mainly venules and veins ). It has an antianginal and antispasmodic effect, relaxes the smooth muscles of the vascular walls , bronchi , gastrointestinal tract , bile ducts , and ureters . When administered intravenously, it causes a rapid decrease in preload on the heart due to the expansion of peripheral veins . It reduces blood flow to the right atrium , helps to reduce pressure in the "small" circle of blood circulation and regression of symptoms in pulmonary edema , reduces afterload, oxygen demand of the myocardium (by reducing preload, afterload and tension of the ventricle walls due to a decrease in heart volume). Promotes redistribution of coronary blood flow in an area with reduced blood circulation. It has a central inhibitory effect on the sympathetic tone of blood vessels, inhibiting the vascular component of the formation of pain . It causes the expansion of cerebral vessels, which explains the headache during its use. With prolonged or frequent use of nitrates of prolonged action, the development of tolerance to the drug (weakening of the therapeutic effect) is observed. After a break, sensitivity is restored. In order to prevent the occurrence of tolerance, it is recommended to observe a daily, preferably nightly, "nitrate-free interval" for 8-12 hours. In most patients, such therapy is more effective than continuous treatment. When used in sublingual and buccal forms, the angina attack is eliminated after 1.5 minutes, the hemodynamic and antianginal effects last up to 30 minutes and 5 hours, respectively. When applying the ointment, the antianginal effect is observed after 15-60 minutes and lasts 3-4 hours. In tablets, retard can prevent the development of seizures. After ingestion, nitroglycerin in the small intestine is gradually released from the capsule and absorbed; the effect occurs 30-60 minutes after administration and lasts 4-6 hours (during this period in the liver it turns into inactive metabolites ). The transdermal therapeutic system (TTC) is a flat multi-layer system that, after attaching the patch to the skin, provides continuous release of nitroglycerin through a permeable membrane that controls the rate of its release from the drug reservoir. The active substance gradually penetrates the blood vessels of the skin , which ensures its circulation in the circulatory system in relatively constant concentrations for the recommended period of time for application. After 12 hours, TTC releases 10% of the initial nitroglycerin content. Since the same amount of nitroglycerin is constantly released from the TTC for each cmΒ² of surface, the dose received by the patient depends solely on the size of the contact surface of the system. Continuous therapy of TTC can be carried out for those patients for whom the clinical effect of the drug for a long period of time practically does not change. The effect when using TTC occurs after 0.5-3 hours and lasts up to 8-10 hours.
Pharmacokinetics
Quickly and completely absorbed from the surface of the mucous membranes and through the skin. When administered, the bioavailability is not more than 10% compared with sublingual nitroglycerin (the effect of the "first passage" through the liver). With sublingual, buccal and intravenous administration, βprimaryβ hepatic degradation is excluded - nitroglycerin immediately enters the systemic circulation . With a sublingual dose of 0.5 mg, bioavailability is 100%, TCmax is 5 minutes. It has a very large volume of distribution. Communication with plasma proteins - 60%. It is rapidly metabolized with the participation of nitrate reductase , with the formation of di- and mononitrates (only isosorbide-5-mononitrate is active), the final metabolite is glycerin . It is excreted by the kidneys in the form of metabolites. The total clearance is 25-30 l / min, and the half-life is 4-5 minutes. With intravenous administration of T1 / 2 - 1-3 minutes, the total clearance - 30-78 l / min. 2 hours after the application of one TTS patch, the plasma concentration of nitroglycerin is reached, which remains during the recommended period of use. The plasma concentration is directly dependent on the area of ββthe contact surface of the TTC and practically does not depend on the place of application of the patch (skin of the chest, shoulder or abdomen). Repeated applications of TTC do not lead to the accumulation of nitroglycerin. The active substance is rapidly metabolized in the liver by glutathione reductase , which affects organic nitrates. In addition, nitroglycerin is metabolized in erythrocytes by enzymatic reactions that occur with the participation of sulfhydryl radicals, as well as in interaction with reduced Hb. Due to the short T1 / 2 during the first hour after TTC removal, the plasma concentration of nitroglycerin falls below the detection limits.
Application

For all forms (comparing risk and benefit) - hemorrhagic stroke , intracranial hypertension, acute myocardial infarction with low left ventricular filling pressure (risk of lowering blood pressure and tachycardia , which can increase ischemia), chronic heart failure with low left ventricular filling pressure, glaucoma (risk of increased intraocular pressure ), severe anemia , thyrotoxicosis , hypotension with a low systolic blood pressure (can exacerbate the condition, causing paradoksal hydrochloric bradycardia and angina), HCM (possibly increasing angina), severe renal failure , hepatic failure (risk of methemoglobinemia ), pregnancy , during lactation , age under 18 years (safety have not been established).
For prolonged oral dosage forms (optional) - increased peristalsis of the gastrointestinal tract , malabsorption syndrome .
For intravenous administration (optional): TBI (increases intracranial pressure), cardiac tamponade , constrictive pericarditis , hypovolemia (should be adjusted before using nitroglycerin - risk of a pronounced decrease in blood pressure), toxic pulmonary edema, atherosclerosis [2] .
Side Effects
From the cardiovascular system: dizziness , headache , tachycardia , flushing of the skin , fever, decreased blood pressure; rarely (mainly with an overdose) - orthostatic collapse , cyanosis .
From the digestive system: dry mouth, nausea , vomiting , abdominal pain.
On the part of the central nervous system: rarely (mainly with an overdose) - anxiety, psychotic reactions, lethargy, disorientation.
Allergic reactions: rarely - skin rash, itching .
Local reactions: flushing of the skin, itching, burning, allergic contact dermatitis .
Other: blurred vision, weakness, hypothermia , methemoglobinemia [2] .

One nitroglycerin-containing combination drug under the Karniland brand name is also registered.
