Story
Detective Joseph Thorne is investigating the mysterious murder of his classmate Jay Cho. At the crime scene, he finds a strange casket and a finger of the child (fingers then turn out to be at the scene of each murder).
In his free time, the detective has fun in a motel with a prostitute Daphne. In the bathroom, he opens the casket and finds himself in a strange place where he meets the Senobits . In the morning, Joseph thinks this is just a nightmare, but at work Daphne calls him and asks for help. Returning to the motel, the detective finds the girl hanged on a hook in the bathroom. To avert suspicion, Joseph leaves an empty cigarette pack with his partner’s fingerprints, Tony Ninonin.
An investigation into the first case leads Thorne to the piercing parlor. From the master, he learns about a certain Engineer with whom Jay Cho had some common business. Joseph goes to his informant, drug dealer Bernie, who is covering up, and demands information from him about the Engineer. Bernie at first refuses to answer, but after beating reports everything that he knows and promises to find out the details.
Ninonin urges to report Daphne to the management, and in response, Joseph talks about the things his partner found at the crime scene. Tony realizes that Joseph set him up, and is forced to remain silent. Soon, the detective receives a videotape on which the faceless Senobit brutally kills Bernie. But when Joseph decides to show the recording to colleagues, the tape is empty. However, Bernie is found dead exactly as it was on tape. From the record on the telephone answering machine of the dealer, Thorne finds out a new address - the gambling house of Parmaji. However, he is severely beaten there and reported that he plays the game of the Engineer.
The battered Thorne seeks help from Dr. Paul Gregory, a psychologist and priest at the Episcopal Church . Mentioning the Lemarshan's box and the Senobits, the doctor tells him about another detective who also investigated the case of the Engineer, but, failing to catch the criminal, committed suicide.
At home, Joseph learns from his wife that someone came to his mother. The detective hurries to his mother and father to a nursing home, and there he is visited by terrible visions. Waking up, Thorne realizes that he is at home, but his wife again informs him of his mother’s call. Joseph does not find his parents in the nursing home, but he notices his father's bloodied bed, next to which he finds a new message from the Engineer with the address.
Arriving at the indicated location, the detective sees how in the house on the other side of the street the Senobit kills his partner Tony. At this time, the bell rings and Joseph learns that there will be four more murders, after which a voice in the telephone receiver advises Thorne to go home. Once there, the detective sees the bodies of his wife and daughter hanging on a pole with nails. Dr. Gregory is here and tells him that, as the police found out, the fingerprints of the child belong to detective Joseph Thorne.
The doctor offers the detective to go to his childhood home. There he sees himself as a child, and his parents as young. Then parents instantly age and try to kill Joseph. He barely manages to escape, after which the detective sees Daphne, Bernie and Tony, who are also trying to kill him. Suddenly, Senobits appear, led by Pinhead. Seeing the Engineer among them, Joseph tries to shoot him, but Pinhead immobilizes him. At this moment, the Engineer tears off the skin from his head, and the detective sees himself in him. Pinhead explains to the shocked Joseph that during the capture of the Engineer, he did not catch the criminal, but revealed the dark side of his own self: indulging in the past vile desires, Thorne embarked on the path of vice, causing his relatives and friends to suffer, and now he will have to pay for it.
After that, the detective wakes up again in the motel’s bathroom next to the casket. Assuming that everything is in order, Joseph calmly goes to work, but there Daphne calls him again. In desperation, the detective commits suicide, but after that he once again finds himself in the bathroom of the motel. The nightmare is endless.
